A Self Direction Mentor (DSP-Self Direction) is responsible to develop and plan services alongside an individual’s self-directed service plans, in compliance with state, federal and agency regulations and operating standards. The Self Direction Mentor works directly with individuals with developmental disabilities to plan and provide habilitative (skill building) or respite support as determined by the Self Direction budget and the individual’s person-centered support team. The Self Direction Mentor will work to increase skill level and increase independence of individuals with developmental disabilities by addressing goals outlined in their individual self-direction plan. The Self Direction Mentor will provide services within the person’s home and in their community to promote independence, inclusion, and quality of life. The Self Direction Mentor is paid from the individuals personal Medicaid budget. The Self Direction Mentors rate of pay is established by the individual.

Requirements

  • Must be 18 years of age.

  • Must possess a valid driver’s license and reliable transportation.
  • Background checks required.
  • Must demonstrate sound decision-making skills, good interpersonal skills, demonstrate understanding of Kelberman’s mission and demonstrate patience & compassion. People working in this position must believe in the philosophy of person-centered support, self-advocacy, and the right and ability of people we serve to lead self-directed lives. The individual in this position must be able to tolerate a moderate noise level in the work environment, handle distractions, and while performing this job may be required to sit, stand, use hands, reach, talk, hear, walk, or lift up to 50 pounds.
